Please DO NOT let anyone in the building. They must use the Ring System in the front breezeway.  If you see someone trying to come in, let them know that the front desk will be out to greet them in a moment. Then return to your desk and let me know that there is a visitor in the front of the building. This applies to parents, families, vendors, and students arriving late. 

EVERYONE MUST GO THROUGH A SCREENING. 

Please follow this for everyone's safety. 

If you have a visitor coming to campus, even to drop something off, alert me with a phone call or email. And if they are staying for a meeting or staying on campus have them complete this FORM. Questions? Contact Cara Garcia

1.  Complete the Following Accident Report form by clicking HERE

2.  Injury Report Insurance form must be filled out.  Make 2 copies, give one copy directly to the injured person/parents, and give a copy to Ms. Birmingham or Trainer if Athletic Injury.   The insurance form can be found at the bottom of this page.
