Medical Clearance

Per school policy, student-athletes must complete online registration through SportsNet and upload a Medical Clearance Form signed by a physician prior to participation in tryouts or practices.

Online registration and medical clearance must be processed by these dates for each season. Each athlete is only required to register once per year. Athletic clearance submitted after these deadlines may not get processed in time for team tryouts.

Clearance Deadlines for 2022-2023 Athletic Registration

SportsNet Registration for the 2022 - 2023 school year will begin July 1st, 2022.  Please submit all athletic medical clearance items by the following dates for your student's respective sport season. 

Fall: August 1st, 2022

Winter: October 24th, 2022

Spring: January 23rd, 2023

Please do not email any medical clearance forms to the the school office. We do not accept hard copies of physicals. All forms are to be completed and attached through our online system.

Riordan Team Physician: Dr. Alan Zhang

Dr. Alan Zhang Headshot

Dr. Alan Zhang is an orthopedic surgeon specializing in sports medicine and minimally invasive arthroscopic surgery of the hip, knee and shoulder. His areas of specialization include tears of the hip and shoulder labrum (cartilage in the socket that stabilizes the joint), femoroacetabular impingement (a painful condition of the hip joint) and athletic injuries of the hip. He also specializes in tears of the knee, as well as rotator cuff tears, shoulder dislocations and biceps tears.

Zhang directs the UCSF Sports Medicine Fellowship Program and the Hip Preservation Center.

After earning his medical degree from the University of California, San Diego School of Medicine, Zhang completed a residency in orthopedic surgery at Ronald Reagan UCLA Medical Center. He completed a fellowship in sports medicine at UCSF, where he now resides as an orthopedic surgeon.
