Print the Prep
Shop Order Form and fax or mail the completed form to
the shop. For "pick-up" orders, please state when during store hours you
would like to pick up and pay for your order. To have items shipped within
seven (7) business days, please include payment (check or money orders only) and all shipping information including a contact phone number. Checks should be made out to: ARHS Prep Shop
